MEDICAL RECORDS REQUESTS

Complete the form below to request access to your medical records from Keystone Medical Family Clinic.

  • Provision of medical records is not covered by OHIP and you will be charged a standard fee based on your preference of either:
    (1) your complete medical records on file - $35.00; OR
    (2) a summary of your medical records on file - $35.00 deposit required for first 5 minutes of physician’s time, billed by the minute thereafter (approximately $8.00 per minute); summary will be provided to patient once remaining outstanding balance payment is received (amount owing for time spent by physician preparing the summary after the first 5 minutes; estimate and detailed invoice will be provided)

  • If you have already paid for your medical records but your secure link has expired (greater than 30 days since link was issued), please e-transfer $5.00 after resubmitting this form.

  • All medical records will be provided electronically in the form of a secure link, as our physical office location has permanently closed and staff is working virtually.

  • You will be provided with secure electronic access to your medical records for a period of 30 days following delivery of your records, during which time you may view, download, or print your file as you see fit.

  • Approximate turnaround time for medical records is 2-4 weeks from the date that payment is received, but volume of requests received may impact this estimate.

  • In the unlikely event that you have not received an email with your secure link to access your medical records within the above timeframe, please wait at least 1 month from your date of payment before sending in an additional follow-up request via this portal. 

  • Urgent requests for medical records will not be accommodated.

  • Payment must be received via e-transfer before staff begins preparing your records. Medical records will not be processed until payment via e-transfer is received, even if a request through this portal has been received.

  • Directions on where to send e-transfer payments will appear at the bottom of this page after clicking the red “SUBMIT” button.
